Zombies in No More Room in Hell 2 are not one enemy with a health bar. They come in several types with different health, different speed and different weak points — and there is a second category of thing you will see that is not an enemy at all, but your own infection talking.
Zombie health values

Standard zombies have a stagger value of 20, which is why light hits from weak weapons interrupt nothing.
Remember that Beginner difficulty reduces effective zombie health by 30%, which players measured independently and the patch notes confirm. The numbers above are the baseline, not what you face on the lowest tier.
Primes
Primes are the elite you need to identify on sight. They are covered in blood with glowing yellow eyes, and they have more health and hit harder than anything else in the standard roster.
Two things make them dangerous beyond the stat line:
- There is a Runner version of a Prime. A 130-health enemy that sprints is a different problem to a 130-health enemy that shuffles.
- They get much more common as difficulty rises. On Nightmare, Runners and Primes appear noticeably more often, and the game adjusts composition dynamically as a match runs.
The same enemy is described elsewhere as a "red zombie" or primary infected, including a red sprinter that makes an "alpha" throat sound. Blood-covered and red are two descriptions of the same thing — do not read them as two separate enemy types and do not plan for a threat that does not exist.
Professional zombies
Zombies retain what they were wearing when they turned, and it matters mechanically:
- Construction workers
- Riot police — these carry shields
- National Guard
- Firefighters
Helmeted zombies resist headshots. Against them, the headshot economy that carries the rest of your play breaks down, and body damage or dismemberment is the better route.
The gas cloud
A freshly killed Shambler leaves a cloud of gas, and standing in it infects you. This is one of the three infection routes in the game and the one most easily avoided — kill Shamblers and move, do not loot over the corpse.
It gets worse: a zombie that has been through the gas has a significantly higher chance of infecting you when it hits you. So the cloud is both a hazard in itself and something that upgrades every zombie that walks through it.
Hallucinations
This is the part most guides get structurally wrong. Hallucinations are not a separate system, a collectible set, or an enemy type. They are a symptom of late-stage infection. If you are hallucinating, you are infected and running out of time.
What you actually see and hear
Players report a consistent set:
- Cockroaches swarming across the screen and the ground
- Shadow demons
- Visual distortion of the picture itself
- Human voices
- Warped, detuned music
Whether the cockroaches are strictly hallucinations is genuinely unsettled — players in the same area report teammates seeing them when they do not, and the community has not resolved it.
The timing is the useful part
Infection runs in three stages: incubation with no symptoms at all, then an onset stage, then the terminal stage. The onset stage brings white light and blur at the edges of vision, audio echo and distortion, and coughing your teammates can hear over voice.
The critical marker is the red veins around the edge of the screen, which appear only in the final 30 seconds.
That gives you a three-step readout you can act on:
Coughing → hallucinations → red veins.
Red veins mean seconds, not minutes. It is also the correct moment to take Phalanx Pills, because each dose resets the timer from wherever you are — taking them at the first cough wastes most of the bottle.
One consequence worth internalising: once you see veins, it is too late to start making Experimental Gene Therapy. Producing the cure takes time you no longer have. At that point you want pills, not a cure.
Reading it on a teammate
You can diagnose someone else without them saying anything. They cough, and their character voice distorts. The heavier the cough and the more pronounced the distortion, the further along the timer is. If a squadmate sounds wrong, ask before they turn — an infected teammate that turns becomes a sprinting zombie that attacks your squad.
Location-specific hallucinations
There are hallucinations unique to particular places rather than to your infection generally. On the map that requires collecting four C4 charges to open a door, an infected player can hear someone singing in the distance. Follow the sound to a bathroom and you will see an apparition sitting on a bench, which stops singing as you approach.
There is no reward for finding these and no collection to complete. They are atmosphere — but they are also confirmation that you are deep enough into an infection to be seeing things.
